Hanrahan Health are excited to announce that we are expanding the range of supports we provide to our clients by adding positive behaviour support!
We are looking for an experienced Behaviour Support Practitioner with current suitability at the Proficient or Advanced level who is driven to provide an exceptional standard of evidence-based, person-centred and outcome-focused positive behaviour support to NDIS participants.
On a daily basis you will be:
- Assisting NDIS participants, along with their families and/or carers, to identify their needs through assessment and person-centred planning
- Evaluating, developing and delivering evidence-based support plans within a complex support space
- Developing meaningful goals and practical strategies utilising a positive behaviour support framework, and with a commitment to eliminate restrictive practices, in the home and community
- Ensuring individual plans reflect our NDIS participants' goals, passions and strengths
- Building the knowledge, capacity and strengths of those who support participants in their lives - family, carers, friends, etc
- Working collaboratively within a multidisciplinary team, including mentoring less experienced Behaviour Support Practitioners.

About you
- Current suitability at the Proficient or Advanced level as a Behaviour Support Practitioner is essential and MUST be able to provide the documentary evidence to prove this
- Considerable experience providing support to people with behaviours of concern (professionally and/or personally) is essential
- Flawless report writing skills
- Excellent communication (written and verbal) skills and interpersonal skills
- Valid drivers licence and access to a reliable motor vehicle
- Genuine passion for working in the disability sector
